What is Case Management?

Case management helps organize the moving pieces of a vocational or disability-related matter so the case can move forward with less confusion and more direction.

At Emerge Vocational Solutions, case management may include reviewing records, understanding work limitations, coordinating vocational needs and medical care, identifying service options, and helping clarify what steps may be appropriate next.

This service is perfect for the busy or overwhelmed caregiver to ensure the injured and/or disabled person does not get lost or overlooked for care.

The goal is to provide steady, practical support while keeping the focus on real-world work ability, available resources, and the person’s next best step.

How the Case Management Process Works

The process is personalized to the case, but often includes:
Initial Case Review

We review the available information, including medical records, work history, restrictions, case concerns, and current vocational needs.

Needs & Goal Identification

We identify what support may be needed, whether that involves return-to-work planning, job placement support, vocational guidance, or service coordination.

Vocational Coordination

We help organize the vocational pieces of the case, communicate clearly about next steps, and support the process with practical recommendations.

Ongoing Case Support

We provide guidance, updates, and case-specific support to help the individual or case stakeholder stay informed and moving in the right direction.

This service is designed to bring structure to the process and help clarify what options may be realistic based on the person’s limitations, goals, and circumstances.

Supportive, Case Specific Process

Case management is tailored to the individual, the records, and the vocational issues involved.

Certified Vocational Expert

Evaluations are conducted by a Certified Rehabilitation Counselor (CRC) and Certified Disability Management Specialist (CDMS).

Is Case Management Right for You?

Case management may be right for you if a vocational matter needs more coordination, follow-through, or practical support than a one-time evaluation or report can provide.

This service can be especially helpful when someone is trying to return to work, explore suitable employment options, understand how limitations affect next steps, or coordinate vocational services after an injury or disability.

For attorneys and case stakeholders, case management can provide a clearer picture of what is happening vocationally and what support may be needed. For individuals, it can offer guidance from someone who understands work capacity, disability, job placement, and real-world employment barriers.

Every situation is different. Emerge Vocational Solutions starts with an honest consultation to determine whether case management is the right fit.
